New Client Process
To become a new client, complete an intake form below and our Client Care Team will contact you within two-three business days. Here at Life Resources, we pair clients with a therapist that best suits the specific needs of the client- such as location, insurance, etc. Our therapists are all licensed and specialized in specific areas that aid our client in their healing process.
If you are unable to complete the form online, please call the office at 843-884-3888.
All new clients are required to pay a non-refundable $25 registration fee.

Adult Counseling Intake Form (Clients that are 18 years old or older.)

Fees
Individual fees vary per therapist & service.
Payment is due at time of service and credit cards are required to keep on file per our cancellation policy and will be used for payment of sessions. Our current payment processor is Square.
Cancellation Policy
A minimum of 24 hours’ notice in advance is required. Please note, Insurance companies will not cover missed or cancelled sessions. In these cases, the client will be responsible for the full session fee. Clients will be responsible for all bank fees as well as the original amount of service for returned checks.
